Support & Wellness is the direct service branch of Breaking Limits. It offers disability support services that are structured, purposeful and built to actually work in everyday life.
Support is delivered through three clear service streams: Social Work Therapeutic Support, Peer Support and Training for Carers & Parents.
Across all services, the focus is consistent — supporting wellbeing, strengthening participation and building the confidence and agency to navigate life whilst working toward your goals in a way that feels more manageable and self-directed.
Everything is delivered via telehealth, making it accessible, flexible and easy to fit into your routine.

People with disability
If you’re looking for services that help strengthen confidence, support participation and build momentum toward your goals, this is designed for you.
Carers and parents
If you support someone with disability and want practical guidance that strengthens confidence, consistency and the support you provide, there is a dedicated service stream for that.
NDIS participants and private clients
Services are available as NDIS disability support for self-managed and plan-managed participants, as well as private-paying options where the service is the right fit.
Social Work Therapeutic Support
Social Work Therapeutic Support is a structured social work service delivered by a qualified social worker.
It is designed to support wellbeing, participation and functioning by working through the real impact of disability in day-to-day life. This might include adjustment, life transitions, relationships, self-advocacy or navigating barriers that make participation more difficult.
Sessions are purposeful and guided, with a clear focus aligned with your individual goals and what you want to move forward.
This may support you with:
navigating life transitions and changes in circumstances
managing disability-related stress and adjustment
building confidence and self-understanding
working through barriers to participation
strengthening self-advocacy and communication
developing practical strategies that support your goals
Best fit when:
you are looking for structured therapeutic support
the focus is on wellbeing, coping, adjustment or participation
you want support that connects clearly to your goals and everyday life
Not the right fit when:
the main need is crisis or emergency support
more intensive or non-disability specific mental health support is required
support is primarily medical in nature
the focus is peer mentoring or informal support
the primary need is carer training or education
Peer Support
Peer Support is a structured, non-therapeutic disability support service grounded in lived experience.
It is designed to support confidence, participation and self-direction by offering practical, disability-informed support from someone who understands the realities of navigating everyday life with disability.
Sessions are reflective and practical, with a clear focus aligned with your individual goals and the steps you want to take forward.
This may support you with:
building confidence and self-direction
strengthening self-advocacy and communication
navigating appointments, services and everyday systems
preparing for participation in community, work or study
reflecting on choices, barriers and next steps
building practical strategies for everyday life
Best fit when:
you are looking for non-therapeutic, practical support
lived experience and shared understanding are important to you
the focus is on confidence, participation and everyday decision-making
Not the right fit when:
the main need is therapy or counselling
there is a need for crisis or mental health intervention
formal support coordination or case management is required
the focus is carer or parent training
Training for Carers & Parents
Training for Carers & Parents is a structured disability support service for parents, carers and other regular support people.
It is designed to strengthen confidence, consistency and practical support approaches in everyday life. The focus is on supporting the person with disability more effectively by strengthening the knowledge and capability of those around them.
Sessions are practical and guided, with a clear focus aligned with the goals, needs and support context of the person with disability.
This may support you with:
building confidence in your support role
strengthening day-to-day routines and approaches
improving consistency in how support is provided
understanding participation barriers more clearly
developing practical strategies that support wellbeing
creating a more stable and supportive environment
Best fit when:
the main focus is supporting a parent or carer in their role
you want structured guidance, coaching or practical strategies
the goal is to strengthen the support environment around the person
Not the right fit when:
the main need is therapy or counselling
family therapy or relationship work is required
specialist behaviour support is needed
the primary need is participant-focused peer or therapeutic support
How it works
From first contact to your first session, the process is designed to be clear, simple and supportive.
Step 1: Fill out the referral form
Start by completing the referral form linked here and share a few details about what you’re looking for.
Step 2: We review the best fit
We look at your goals, circumstances and support needs to determine the most appropriate service pathway.
Step 3: We confirm next steps
Once the right pathway has been identified, we move through intake, consent, service agreement and practical setup.
Step 4: Support begins!
Sessions begin via telehealth with a clear focus aligned with your individual goals.
Funding and delivery
Support & Wellness services are delivered through telehealth disability support, making it easier to access structured, consistent support wherever you are.
As Breaking Limits is not currently an NDIS registered provider, services are available for:
self-managed NDIS participants
plan-managed NDIS participants
private fee-paying clients
For NDIS-funded services, the specific support pathway is confirmed based on individual goals, support needs and the type of service being delivered. For private clients, the same service structure and boundaries apply, even though NDIS claiming rules do not.
